Tenaga Kesehatan sebagai Sumber Stigma dalam Pelayanan Kesehatan: Perspektif Antropologi Kesehatan
Main Article Content
Abstract
Health-related stigma has become an important determinant of healthcare quality by limiting access to healthcare services, weakening therapeutic relationships, and reducing treatment adherence. While stigma has traditionally been viewed as negative attitudes held by society toward particular groups, the role of healthcare professionals as potential sources of stigma has received comparatively less attention. This article aims to analyze the factors that lead healthcare workers to become a source of stigma within healthcare services, explain the impact of this on the quality of healthcare, and formulate a conceptual model for stigma reduction through the perspective of medical anthropology. The study employed a narrative literature review by synthesizing evidence from peer-reviewed articles, academic books, and international policy documents concerning health-related stigma, health anthropology, social psychology, organizational culture, and healthcare quality. The findings indicate that stigma emerges through the interaction of healthcare professionals' implicit bias, social constructions of illness, organizational culture, and healthcare system factors. These interactions reduce healthcare access, weaken therapeutic communication, decrease treatment adherence, increase patient safety risks, and widen health inequities. This article proposes a Multilevel Transformation Model for Stigma-Free Healthcare, emphasizing simultaneous transformation at the individual, organizational, and healthcare system levels. The proposed model provides a conceptual foundation for developing policies, health professional education, and quality improvement initiatives aimed at creating more inclusive and patient-centered healthcare services.
